
| Disabled Parking Permit | |

|
A parking permit is available to individuals with permanent mobility problems or certification as legally blind. The disability must be certified by one of the following: |

A licensed Physician, Osteopath, Podiatrist, Chiropractor or Optometrist. |
|
|
The Adjudication Office of the U.S. Department of Veterans Affairs, the Veteran’s Administration, or the Florida Department of Veterans Affairs. |
|
|
The disabled person and the certifying authority must complete an application form (83039) dated June 1998 or later. |
|
|
In order to qualify, disabled persons must have either a current Florida Driver’s License or a Florida Identification Card issued by the Florida Department of Motor Vehicles. |
|
|
A placard will be issued for a period of up to 4 years and will expire on the disabled person’s birthday. This placard is provided at no charge. |

|
A wheelchair symbol license plate may be issued to a disabled person who owns a vehicle and qualifies for a disabled parking permit. A disabled veteran who has a DV license plate and qualifies for a parking permit may also be issued a DV wheelchair plate. |
| Florida has a reciprocal agreement with every state in the United States which allows a resident from another state to use their current permit from their home state on any vehicle they are riding in while visiting Florida. There are also 12 provinces in Canada that are reciprocal with Florida. Please call our office for those 12 provinces. |
